How to Configure and Use the Insurance Declaration Form

The Insurance Declaration Form allows customers to review the relevant insurance documents, select their preferred level of cover, declare the value of their goods, list valuable items, and submit a signed insurance request.

This guide explains how to configure the form in MoverMate, send it to customers, and understand the customer submission process.


Configure the Insurance Declaration Form

Navigate to:

Settings → Insurance Declaration Form

From this page, you can configure:

  • Terms & Conditions Links

  • Blanket Cover Range

  • Default Room Templates

  • Thank You Message



Configure Terms & Conditions Links

The Terms & Conditions Links section controls the insurance documents displayed to customers before they begin the form.

These documents may include:

  • Product Disclosure Statement (PDS)

  • Supplementary Product Disclosure Statement (SPDS)

  • Financial Services Guide

  • Target Market Determination (TMD)

To update the links:

  1. Click Edit in the Terms & Conditions Links section.

  2. Enter the document name.

  3. Enter the public URL for the document.

  4. Click Add Link to include another document.

  5. Use the delete icon to remove a document.

  6. Click Save.

Customers must acknowledge that they have read and agreed to the applicable documents before continuing.


Note: Make sure each URL opens the correct document and can be accessed without requiring a login.



Configure the Blanket Cover Range

The Blanket Cover Range determines the values available when a customer selects blanket cover.

You can configure:

  • Minimum cover value

  • Maximum cover value

  • Step amount

For example:

  • Minimum value: $20,000

  • Maximum value: $500,000

  • Step: $5,000

To update the range:

  1. Click Edit.

  2. Enter the minimum value.

  3. Enter the maximum value.

  4. Enter the step amount.

  5. Click Save.

Customers can then use the value field or slider to declare the total replacement value of the goods being moved.




Configure Default Room Templates

The Default Room Templates section controls the rooms and items shown when a customer selects itemised cover.

Default rooms may include:

  • Study

  • General

  • Hallway

  • Kitchen

  • Bedrooms

  • Dining Room

  • Family Room

  • Lounge Room

  • Bathroom/Laundry

Each room can contain a list of commonly moved items.

To update the room templates:

  1. Click Edit in the Default Room Templates section.

  2. Update a room name where required.

  3. Click Add Item to add another item to a room.

  4. Use the delete icon beside an item to remove it.

  5. Click Add Room to create another room template.

  6. Click Save.

These templates make it easier for customers to provide an itemised declaration without manually creating every room and item.



Configure the Thank You Message

The Thank You Message is displayed after the customer successfully submits the form.

You can customise the:

  • Title

  • Subheading

Example:

Title

Thank You for Your Insurance Declaration

Subheading

Your insurance declaration has been submitted successfully. Please contact us if you need to update any information.

Click Save after making your changes.

The confirmation page also displays the customer’s reference number and your business contact details.



Send the Insurance Declaration Form

The form can be sent to customers through an email template using the following merge field:

{{Customer_Insurance_Url}}

This merge field generates the customer’s unique Insurance Declaration Form link.





How Customers Complete the Form

When the customer opens the link, they are guided through the following steps.

Step 1: Review the Insurance Documents

The customer is shown the documents configured under Terms & Conditions Links.

They must confirm that they have read and agreed to the terms and conditions before clicking Continue.


Step 2: Choose the Level of Cover

The customer can choose between:

Itemised Cover

The customer enters the replacement value of individual items within each room.

Blanket Cover

The customer enters the total replacement value of the goods being moved without listing every standard household item separately.

Valuable items can still be declared separately.

Step 3: Confirm Interstate and Storage Requirements

The customer can indicate whether:

  • The move is interstate.

  • Extended cover is required for storage.

The form displays additional information about storage cover and any applicable standard storage period.


Step 4: Enter the Value of the Goods

The information requested depends on the selected cover type.




Blanket Cover

The customer:

  1. Enters the total replacement value of the goods being moved.

  2. Uses the amount field or slider to adjust the value.

  3. Adds any valuable items separately.

  4. Clicks Continue.



​Itemised Cover

The customer:

  1. Expands the relevant room.

  2. Enters a replacement value beside each item to be insured.

  3. Leaves items that are not required at $0.

  4. Clicks Add Item to include an item that is not listed.

  5. Adds another room where required.

  6. Adds valuable items separately.

  7. Clicks Continue.




Step 5: Review the Declaration

Before submitting the form, the customer can review:

  • Policyholder details

  • Selected cover type

  • Interstate move selection

  • Storage extension selection

  • Value of standard goods

  • Number of declared items

  • Value of valuable items

  • Total value of goods to be insured

The customer can use the available Edit options to make changes before submission.

The review page also explains that an insurance quotation may be provided based on the submitted information.


Step 6: Sign the Declaration

The customer must provide a signature before submitting the request.

They can either:

  • Draw their signature, or

  • Type their name and use the generated signature

The customer must also confirm that the information supplied is true and correct.


Step 7: Submit the Insurance Request

Once the information has been reviewed and signed, the customer clicks:

Submit Insurance Request

After submission, the customer sees the configured Thank You Message.

The confirmation page includes:

  • Submission confirmation

  • Insurance declaration reference number

  • Business phone number

  • Business email address

  • Instructions for requesting changes

Customers should quote the displayed reference number when contacting the business about their declaration.



Important Information

  • Confirm that all insurance document links are correct and publicly accessible.

  • Configure the blanket cover range according to the insurance product being offered.

  • Review the default room and item templates before sending the form.

  • Customers should enter replacement values where required by the applicable insurance documents.

  • Customers should contact the moving company if information needs to be changed after submission.

  • Submitting the Insurance Declaration Form does not automatically activate insurance cover. A quotation may still need to be issued and accepted.
